Intake Form

Front Desk

When you walk in to our office, one of our patient care coordinators will get you set up and gather all the information we need to develop the personalized treatment plan that's right for you. We love meeting new patients and helping them settle in. Bring your medical and medication information so we can consider all health factors. To speed things up ... just fill out our intake form. What works for you, works for us!

Treatment Coordination

Have important questions about your family’s dental care? We will answer them – especially when it comes to insurance (we accept most plans), financing needs, and the cost of your treatment. You shouldn’t have to figure it all out on your own … and we don’t expect you to. We will take the time to get to know you – and work with you – to make your experience a good one.

Hygiene Care

When you stop in for your first appointment, we may spend more time looking at your mouth than we would at subsequent visits. That’s because we need to learn everything we can about your smile and gain a clear understanding of your oral health needs and concerns. We’ll look for any jaw joint problems and screen for oral cancer. This is a great time for you to ask any questions you might have about your at-home cleaning routine. Our knowledgeable registered hygienists will provide important tips to help you look after your smile.

Practice Tour

Feeling uneasy about your first visit? If that’s the case, contact us to arrange a full tour of our street-level office. Explore our reception area with comfortable seating, beverage bar, and the soothing scents of essential oils in the room. You’ll get to know us and we’ll get to know you, and you’ll feel much better about your upcoming appointment. And if English isn’t your first language, don’t worry … we speak French, Romanian, and Spanish. Did you know? Each year we host a customer appreciation wine and cheese party at our office! We hope to see you there.

Checkup

It’s important that when you meet the doctor, you discuss your personal oral health needs and desires. You will learn what our exam reveals while we work with you to determine the personalized approach that works best for you. Whether we set up a regular care schedule, design a treatment plan to address decay or gum issues, or discuss your cosmetic concerns, this is your chance to tell us what you like and don’t like about your smile and general oral health. Then together, we will meet mutually established goals.
